Quick answer: Meta ads work for Indian hospitals and clinics when you advertise the service (not the patient's condition), use a Higher intent lead form with 2–3 non-medical qualifying questions, call or WhatsApp every enquiry within minutes, and send lead quality back to Meta through the Conversions API (Meta CAPI) so its AI looks for people who actually book appointments. Meta's health ad policies and any Indian medical-advertising rules that apply to you come first. Check them before every campaign.

Facebook marketing is one of the cheapest ways for a hospital, a dental, skin or hair clinic, or a diagnostics centre in India to get appointment enquiries. It is also where many clinics get stuck with rejected ads, wrong numbers and enquiries nobody calls back. This guide covers the full lead-ad workflow for healthcare in 2027: campaign setup, compliant creatives and targeting, lead form design, speed-to-lead, and how to teach Meta which leads became patients.

This is marketing guidance, not legal or medical advice. Meta's policies and Indian rules for medical advertising change. Confirm the current versions before you launch.

Who this guide is for

  • Hospitals and multi-speciality clinics promoting OPD appointments, health check-up packages or new departments.
  • Dental, skin and hair clinics promoting consultations for elective treatments.
  • Diagnostics centres and labs promoting home sample collection or test packages.
  • Agencies running Meta ads for any of these.

If your goal is phone calls and appointment requests from people near your centre, lead ads (instant forms) are usually the simplest place to start. If your front desk prefers chatting, compare them with Click-to-WhatsApp in lead ads vs messages ads.

Step 1: Know the Meta rules that matter most for healthcare

Most rejected healthcare ads break one of a handful of rules. These are the ones to check first, based on Meta's own policy pages:

  • Don't assert or imply the viewer's health condition. Meta's Privacy Violations and Personal Attributes policy does not allow ads that assert or imply personal attributes, including physical or mental health and medical conditions. Meta's own examples allow "New diabetes treatment available" but not "Do you have diabetes?"
  • Cosmetic, weight-loss and health products and services have extra rules. Meta's Health and Wellness policy requires ads for cosmetic procedures, weight loss and similar health products and services to target people aged 18 or older. It also bans negative statements that attack a person's appearance, body parts or hygiene. Meta has changed its rules on before-and-after images over time, so check the current page before you use them.
  • Lead forms can't ask for health details without permission. Meta's Advertising Standards restrict lead form questions about things like mental health, disabilities and insurance unless Meta has given prior written permission.
  • Health-related targeting is limited. In January 2022 Meta removed detailed targeting options for sensitive topics, including health causes such as "Chemotherapy" or "World Diabetes Day". Don't expect to target people by condition.
  • Health websites may have data-sharing limits. Meta may restrict which events and data a health-related website can share through the Pixel and Conversions API. You can check your data source's status in Events Manager under Settings. Meta explains this in data sharing restrictions based on data source categories.

Indian rules apply on top of Meta's. Doctors are also bound by National Medical Commission (NMC) rules on advertising and self-promotion, health claims fall under the ASCI code, and personal data falls under the Digital Personal Data Protection Act, 2023. Get these checked by someone qualified. Don't assume that because Meta approved an ad, it's allowed under Indian rules.

Step 2: Set up the campaign

  1. Own your assets. Keep the Facebook Page, ad account, Pixel/dataset and lead forms in a Business Portfolio owned by the hospital or clinic. Give your agency partner access, not ownership.
  2. Choose the Leads objective with instant forms. If you will send lead quality back to Meta, use the Conversion leads performance goal so Meta optimises for leads that move forward, not only form fills. See Meta Conversion leads goal and the lead ID.
  3. Target by location first. For most clinics the catchment area is a few kilometres around the centre, or a set of PIN codes or localities. A broad audience within that radius plus a good creative usually beats guessing at interests.
  4. Split by service, not by "brand". A dental implant ad and a general check-up ad bring very different leads. Separate campaigns or ad sets make lead quality much easier to read.
  5. Set a budget you can actually call back. If your front desk can handle 15 calls a day, don't run a budget that brings in 60 leads.

Step 3: Write creatives that get approved and attract the right patients

A safe rule: describe your service, your doctors and your facility, never the viewer's body or illness.

Avoid (implies the viewer's condition)Use instead (describes the service)
"Suffering from hair fall?""Hair and scalp consultations at our Andheri clinic"
"Tired of your crooked teeth?""Clear aligner consultations with an orthodontist, Mon–Sat"
"Is your sugar out of control?""Diabetes care OPD, Tuesdays and Fridays"
"Your skin looks dull and old""Dermatology consultations, book a slot online"
  • Show real people and places. Doctor-led explainer videos, your reception, equipment and team usually build more trust than stock photos.
  • Be factual about what's on offer. Give the timings, location, doctor's qualifications and how booking works. Avoid "best", "guaranteed" or promised results within a set time.
  • Use the same message on the form. The form intro should repeat the service and the location, so people know exactly what they're asking for.

Step 4: Design a lead form that filters junk without asking medical questions

Healthcare forms have to do two jobs: keep out low-intent and accidental submissions, and stay away from health information Meta restricts.

Use Higher intent

The Higher intent form type adds a review step before submitting. That usually means fewer leads, but more of them actually pick up the phone. We compare both in Higher intent vs More volume.

Ask 2–3 non-medical qualifying questions

  • Which service are you interested in? (multiple choice from your own list, e.g. consultation, check-up package, home sample collection)
  • Which branch is convenient?
  • When would you like to visit? This week / next week / just exploring
  • Preferred time for a call back? Morning / afternoon / evening

Leave out symptoms, diagnoses, medications and insurance. Your staff can ask those later, on the call or at intake. More ideas are in 12 qualifying questions that filter junk leads.

Consider appointment requests

Meta's lead forms can include an appointment request step and conditional questions, so people can pick a preferred day or branch. Set up the questions so they suit your slots. See conditional questions and appointment requests.

Step 5: Respond in minutes (speed-to-lead)

People looking for a clinic often send enquiries to several clinics in one evening, and the first one to call usually gets the booking. Facebook doesn't alert your front desk on WhatsApp by itself, so leads often sit unseen in Meta Business Suite.

Marketing automation fixes this. LeadSmart captures your Meta lead-ad leads and sends an instant WhatsApp alert to your business for every new enquiry, so whoever is at the desk sees it immediately. Every lead also syncs to Google Sheets. If you want automatic follow-up messages to the leads themselves, you can build them in your own tools through a webhook. See Facebook lead ads webhook with n8n.

For WhatsApp marketing, keep it simple and with permission: a short first message that says who you are and which enquiry you're replying to gets more replies than repeated unanswered calls. More in WhatsApp marketing for Facebook lead ads and getting lead notifications on your phone.

Step 6: Send lead quality back to Meta (Meta CAPI)

This is where AI marketing actually earns its keep. Meta's delivery system optimises for the signal you give it. If all it ever sees is form submissions, it goes looking for more people who submit forms. Once it sees which leads were reached, qualified and booked, it can look for more people like them.

  1. Agree on simple labels with your front desk: not reachable, junk, not interested, qualified, appointment booked, visited.
  2. Label every lead after the first call.
  3. Map labels to Conversions API events. In LeadSmart you connect your Meta Pixel/dataset and map lead labels to CAPI events without a developer. The events include Meta's lead ID, so they work with the Conversion leads goal.

Keep event and label names neutral. Use stage names like "qualified" or "appointment booked". Don't use names that reveal a treatment or condition. Meta's business tools terms restrict sharing sensitive health information, and Meta may filter or restrict events that look sensitive. Step-by-step setup: Meta CAPI without a developer and sending lead quality back to Meta.

Step 7: Measure cost per booked appointment, not cost per lead

A ₹150 lead that never picks up costs more in the end than a ₹400 lead that books. Track these for each campaign every week:

  • Leads, and % reached on the first call or WhatsApp
  • % qualified
  • % appointment booked and % visited
  • Cost per booked appointment

Use your own numbers. Decision time varies a lot by service: a routine dental check-up books much faster than an elective cosmetic treatment. Judge each campaign over a time frame that suits its service. Export from the LeadSmart dashboard or use the synced Google Sheet as your working list. If you see many fake or wrong numbers, read fake leads from Facebook lead ads.

FAQ

Can hospitals and clinics in India run Facebook and Instagram ads?

Yes. Healthcare providers can advertise on Meta as long as the ads follow Meta's Advertising Standards, especially the personal attributes and health and wellness policies, plus any Indian rules that apply to medical advertising.

Why does Meta reject my clinic's ads?

A common reason is copy that asserts or implies the viewer has a condition, like "Do you have back pain?" Rewrite it to describe your service ("Back pain clinic in Pune, book a consultation"). Also check for missing 18+ targeting on cosmetic or weight-loss services, and for appearance-shaming language.

Can I target people with a specific disease on Facebook?

No, not directly. In 2022 Meta removed detailed targeting options linked to sensitive health topics. Use location, age where appropriate, broad audiences, and lead quality signals so Meta's AI can find better leads.

What questions can a healthcare lead form ask?

Stick to service of interest, preferred branch, preferred visit time and callback time. Meta restricts questions about health, disabilities and insurance without prior permission, so ask those later through your own process.

How do I get better quality leads on Facebook for my clinic?

Use Higher intent forms, ask a few qualifying questions, respond within minutes, label every lead, and send those labels back to Meta through CAPI with the Conversion leads goal.

Does LeadSmart send WhatsApp messages to patients automatically?

No. LeadSmart sends instant WhatsApp alerts to your business. If you want automatic follow-ups to leads, you can trigger them from your own tools, such as n8n, through a webhook.
